Annuity

A list of available forms will be displayed

Withdrawal:  

  1. Partial cash disbursement – form is used when a partial cash disbursement is being requested.  A partial distribution may be restricted by Federal Income Tax laws and the terms of your employer's plan. Additional signatures from a Plan Administrator, Third Party Administrator or Spouse may be required. Contact the Client Care Center at 1.800.448.2542 for additional information. 
  2. Hardship and Unforeseen Emergency – Contact the Client Care Center for information on the requirements and restrictions for this type of request.  May require supporting documentation and additional signatures from a Plan Administrator, Third Party Administrator or Spouse. 
  3. Surrender of the account - form is used when a total distribution from the account is being requested. A total withdrawal from your AIG Retirement Services account may be subject to income taxes, Internal Revenue Service penalties and AIG Retirement Services surrender charges. Additional signatures from a Plan Administrator, Third Party Administrator or Spouse may be required. The ability to surrender your account may be restricted by Federal Income Tax laws and the terms of your employer's plan. Contact the Client Care Center at 1.800.448.2542 for additional information. 


Beneficiary Change – If you want to submit a paper form to update your beneficiaries, this form would be used. 

Name change -  form is used to make administrative changes on your account such as:

  • name change,
  • update address and telephone number,
  • request a correction to the social security number on file ( a copy of the social security card will need to be submitted),
  • request a correction of the date of birth that is on file (a copy of your driver’s license or birth certificate will need to be submitted)
  • for non-qualified deferred annuity (NQDA) ownership change (see form for listed requirements)
  • delivery option election (to go paperless or receive account information in printed format)

You will be directed to the page that has the ePrint icon which is a hyperlink to the employer group’s page where a “Quick Links” menu will be displayed. Click on “Forms” to have the menu of forms that are available to be downloaded and printed will display.
